The relationship between lymphopenia and severity in coronavirus disease 2019 (COVID-19) is one of the most consistent fields to be studied within the new evidence on the new coronavirus. In this context, our research team has carried out a general but concise review of the most recently discovered concepts about the pathological mechanisms of COVID-19, focusing on the pathophysiology behind lymphopenia. After addressing different scientific articles, it was shown that the type 2 coronavirus that causes severe acute respiratory syndrome (SARS-CoV2) influences the immune response of lymphocytes through different mechanisms, and alters their morphology, resulting in a poor immune defense and subsequent progression of the disease towards severity; phenomenon that was studied by many authors who conclude that lymphopenia is a reliable predictor of severity
