To determine whether sociodemographic, clinical, and surgical factors are associated with unfavorable outcomes of appendectomy in patients with acute appendicitis. Methods: An observational, analytical, cross-sectional study was conducted, including all patients over 17 years of age who underwent appendectomy for acute appendicitis at the Regional Teaching Hospital of Trujillo during the mentioned period. The evaluated sample consisted of 162 patients. Sociodemographic, clinical, and surgical information was collected from the patients, and unfavorable outcomes were recorded. A descriptive analysis was performed, and statistical tests, such as the Chi-Square test and Student's t-test, were used for inferential analysis. Additionally, a multivariate analysis was carried out using logistic regression. Results: The prevalence of unfavorable outcomes was 31.48%. Unfavorable outcomes were significantly associated with rural residence, the presence of type 2 diabetes mellitus, a longer duration of illness, a higher leukocyte count, a longer operative time, the presence of a mass at the site of inflammation, perforated appendicitis, and a longer hospital stay. Conclusion: Sociodemographic, clinical, and surgical factors were significantly related to unfavorable outcomes in patients undergoing appendectomy for acute appendicitis. The presence of type 2 diabetes mellitus, a longer duration of illness, and a longer operative time were independent factors that increased the risk of unfavorable outcomes. The choice of open surgery was also associated with a higher risk of unfavorable outcomes
