This research work is quantitative approach, is based on the Adaptation Model of Callista Roy. Its objective was to determine the relationship between the level of information and the degree of anxiety of the patient in the preoperative period in a Private Clinic in Chimbote, 2020. The sample population consisted of 60 patients with scheduled surgery; For data collection, the test on the level of pre-surgical information and the Amsterdam Preoperative Anxiety and Information Scale (APAIS) were used as instruments. The results obtained allowed us to conclude that 50% of the patients obtained a high level of information, 46.7% a medium level and 3.3% a low level of information. Regarding the degree of anxiety, 80% of the patients presented some degree of anxiety, 50% being mild and 30% moderate, the remaining 20% of patients did not present anxiety. Likewise, there is a significant statistical relationship between the level of information and the degree of anxiety [Spearman's Rho = -0.702 gl = 2 p = 0.000]
