Objective: Determine the anatomopathological surgical clinical correlation of patients operated on appendicectomy at the “Hospital Nuestra Señora De las Mercedes” in Paita – Piura from January 2017 to January 2019. Methodology: The present study is a type observational, retrospectively, and descriptive – transversely type. Case histories and anatomopathological reports of patients operated with diagnosis of sharp appendicitis (AA) were checked, which were expiring with the criteria of inclusion and exclusion.Descriptive statistics were used for demography and nonparametric statistics (Kappa index) to analyze the concordance between surgical diagnosis and pathological diagnosis. Results: 313 patients with an average age of 25.54 years, 52.4% are male and 47.6% female. Surgeons diagnosed necrosed AA 36.7%, suppurated 18.8%, perforated 7.7% and congestive 5.8%.The most frequent position was determined to be 63.3% retrocecal, pelvic 17.9%, paracecal 7.6% and 11.2% were not assigned position. Surgeons did not diagnose appendages without inflammatory signs being confirmed by pathological anatomy. Clinical stages of AA by pathological anatomy is 47.6% necrotic, 26.2% suppurated, 13.4% congestive and 12.8% perforated. Anatomopathological study defines the diagnosis of AA, out of 135 exposed by surgeon 111 (35.5%) are uncomplicated appendicitis confirmed and 178 exposed by surgeon 165 (52.7%) are confirmed complicated appendicitis. Conclusions: By categorizing acute apendicitis in stages, a moderate interobserver concordance with a Kappa index was obtained: 0.53 between surgical and anatomopathological finding. By classifying them in complicated and uncomplicated acute appendicitis, a good match was obtained with a Kappa index: 0.75.
