To determine the effectiveness of high ligation compared with low ligation of the IMA in patients diagnosed with adenocarcinoma of the sigmoid, upper and middle rectum operated on in the Oncology Surgery Service. Material and methods: A retrospective cohort study was carried out in which 63 patients diagnosed with adenocarcinoma of the sigmoid, upper and middle rectum were included, according to selection criteria which were divided according to exposure to high ligation or low inferior mesenteric artery ligation, chi-square and 5-year survival were calculated. Results: There are no significant differences regarding demographic, clinical and analytical characteristics between patients who underwent high ligation versus low ligation of the inferior mesenteric artery (p>0.05); There are no significant differences regarding the characteristics of the tumor between patients who underwent high ligation versus low ligation of the inferior mesenteric artery (p>0.05); High ligation is as effective as low ligation of the inferior mesenteric artery in maintaining 5-year survival in patients diagnosed with adenocarcinoma of the sigmoid, superior and middle rectum operated on in the Surgical Oncology service. Conclusion: High ligation is as effective as low ligation of the inferior mesenteric artery in maintaining 5-year survival in patients diagnosed with adenocarcinoma of the sigmoid, superior and middle rectus.
