Objective: Evaluate the utility of the fetal brain-placental index (BPI) to predict adverse perinatal outcomes in gestations of 41 weeks with induced labor. Material and Method: Analytical, retrospective, observational and cross-sectional study of diagnostic tests in 97 pregnant women of 41 weeks attended in the Hospital Regional Docente of Trujillo - Peru from January 2015 to December 2018, in whom the Doppler study was performed the antenatal calculation of the fetal BPI and its prediction was evaluated with adverse perinatal results during the induced labor. Results: With a threshold value of 1.205, under the ROC curve, fetal BPI predicted the presence of APGAR score of less than 7 at 5 minutes with sensitivity of 100%, specificity of 96.84%, with a validity index of 96. , 91% and Likelihood Ratio (LR) positive of 31.67 (95% CI, 10.4 - 96.43). The threshold or cut-off values of fetal BPI to predict the presence of meconium amniotic fluid and cesarean section due to acute intrapartum fetal distress were 1,755 with a positive LR of 1.85 (95% CI, 1.32-2.61) and 1,985 with positive LR of 1,32 (95% CI, 0.98 - 1.78), respectively. Conclusion: Fetal BCI is useful for predicting the APGAR score of less than 7 at 5 minutes, but does not predict the presence of meconium fluid and cesarean delivery due to acute fetal distress in pregnant women of 41 weeks with induced labor.
