Objective: To determine the association of intraoperative factors with prolonged hospital stay in children with secondary peritonitis at the Hospital José Cayetano Heredia, in 2022. Materials and methods: An observational, analytical case-control investigation was carried out. The data was collected at the Cayetano Heredia hospital in the city of Piura, whose population included children from 6 months to 14 years hospitalized in said hospital. Sociodemographic information and both clinical and intraoperative characteristics were collected. To carry out the investigation, the statistical program STATA in its version 16 was used. Results: 123 medical records from the year 2022 were reviewed by non-probabilistic sampling of consecutive cases in children aged 6 months to 14 years with secondary peritonitis. The age with a mean of 8.02+2.91 years, in those who presented fever with a temperature >38°C, a significant relationship was found (p=0.006), likewise those with signs of intestinal obstruction (p=0.007) and resistance to palpation (p=0.002). Those patients with comorbidities were associated with length of stay (p=0.003) given discharge at the physician's discretion, as well as illness time ≥80h (p=0.002). The regression analysis determined that a generalized extent of peritonitis was associated with a 5.17-fold increased risk of prolonging hospital stay (ORa=5.17, 95% CI: 2.25 – 14.51, p<0.001), an operating time >60 min was associated 1.08 times higher risk of prolonging hospital stay (ORa=1.08, 95%CI:1.04-1.12, p<0.001) and open surgery increased the risk 3.53 times (ORa=3.53, 95%CI: 1.35-9.23, p=0.01) being statistically significant results. Conclusions: The intraoperative factors that were associated with a prolongation of hospitalization in pediatric patients with peritonitis were the extent of the peritonitis, the operative time and an open surgery.
