To determine whether the prevalence of non-traumatic cardiac injury exceeds 10% in patients with moderate and severe trauma and to identify its associated factors. Methods: An observational, analytical, cross-sectional study was conducted on a sample of 156 patients aged 18 and older with moderate and severe trauma, excluding direct cardiac trauma, admitted to the Hospital Regional Docente de Trujillo between February 2020 and June 2022. Statistical tests, including the Chi-square test and Student's t-test, were used for data analysis. Demographic and clinical data were collected, and a qualitative troponin test was employed. Results: A prevalence of non-traumatic cardiac injury of 14.74% was found in the study sample. The average age of patients with cardiac injury was 43.91 ± 22.74 years, compared to 32.68 ± 14.58 years in those without cardiac injury. The majority of patients with cardiac injury were male (86.96%). Factors such as RTS and NISS scores, severe trauma according to NISS, serum glucose levels, and the presence of hyperglycemia were significantly associated with non-traumatic cardiac injury. Conclusions: The results suggest that the prevalence of non-traumatic cardiac injury in patients with moderate and severe trauma exceeds 10%. Furthermore, a significant association was observed between cardiac injury and various clinical and trauma-related factors
