Objective: To determine the factors associated with the normal leukocyte count and C-reactive protein, and inflammatory parameters, in adults with acute appendicitis. Methods: A nested case-control study was performed in a cohort. The study population consisted of patients aged 18 to 60 years undergoing surgery for acute appendicitis. 333 medical records were reviewed, selected through convenience sampling. The variables included and analyzed were: leukocyte count, CRP, age, sex, weight, height, origin, self-medication, DM2, HBP, type of appendicitis, duration of illness, preoperative time, type of appendectomy, operative time and hospital stay. Patients were divided into 2 groups: patients with normal inflammatory parameters and patients with abnormal inflammatory parameters. Both groups were compared with the aim of determining which of all the factors included in this study were associated with normal inflammatory parameters. Data were analyzed using SPSS version 28 Results: Of the 333 patients, 11.11% had normal inflammatory parameters. The average age was approximately 33 years for both groups. In the group with normal parameters, 56.76% were male, whereas it was 57.43% in the abnormal group. The mean preoperative time was shorter in the abnormal group, and acute catarrhal appendicitis was more frequent in the normal group as well. The multivariate analysis identified that origin and self-medication were significantly associated with the normality of the inflammatory parameters. Conclusion: The prevalence of normal inflammatory parameters in patients with acute appendicitis was 11.11%. Rural origin, self-medication, preoperative time and catarrhal appendicitis are significantly associated with normal inflammatory parameters in patients with acute appendicitis
