Determine the sociodemographic factors associated with therapeutic adherence in hypertensive adults from the I-4 Canchaque Health Establishment, 2022. Materials and methods: The study was configured as an observational study with a cross-sectional and comparative analytical design. A sample of 260 hypertensive adults with treatment, treated at the I-4 health facility in Canchaque, was shown. The techniques used in the methodology were the data collection sheet supported by the exhaustive review of clinical records; and the use of the Morisky-Green Levine test to evaluate the level of therapeutic adherence. For the analysis, the STATA version 16 program was used, using parametric measures such as chi square to evaluate the association between the variables. This comprehensive approach provided a solid foundation for understanding the association between therapeutic adherence and associated factors. Results: In sociodemographic factors, a statistically significant association was observed with age (p=0.000) with a median of 69 years; the level of education (p=0.013) of which 48.46% achieved secondary education; 30.38% primary studies; 13.08% had higher education and only 8.08% of the patients did not have any kind of education. Occupation had great significance (p=0.000) where the majority were from rural areas at 71.92%. Finally, the duration of illness showed an association with adherence (p=0.001), with 76.92% representing an illness duration greater than 5 years. Conclusions: The conclusions of this study indicate that sociodemographic factors such as age, level of education, occupation and duration of illness show a statistically significant association with therapeutic adherence in hypertensive adults who attend the I-4 Canchaque Health Establishment in 2022. These findings highlight the importance of considering these aspects when designing interventions aimed at improving therapeutic adherence in this population
