The This study aimed to determine the prevalence of dental caries in permanent first molars according to ICDAS II in schoolchildren aged 6 to 12 years in State Educational Institutions of the District of Tayabamba- La Libertad 2019 It was a prospective, cross-sectional, comparative and observational study, including a total of 286 randomized State Educational Institutions in the District of Tayabamba. A clinical examination was performed on each of the schoolchildren, evaluating and recording the state of progression of the carious lesion following the criteria of the ICDAS II system. The results showed that in the National Educational Institutions of the District of Tayabamba they presented a higher prevalence of caries according to age, it is led by schoolchildren of 7 years (23.16%) and schoolchildren who have less carious lesions of 6 years (6.81%) . The highest percentage of ICDAS II with Code 0 was found in 1MSD with 2% and 1MSI with 2%; respectively, Code 1 in the 1MSD with 6%, Code 2 in the 1MSD and 1MSI with 9%; respectively, Code 3 in the 1MII with 3%, Code 4 and 5 in the 1MID with 8% and 2%; respectively, Code 6 and Not erupted in 1MSD, 1MSI, 1MID and 1MII with 1%; respectively. According to gender, the prevalence of dental caries in permanent first molars occurred in men (53.49%) and in women (46.50%), while the codes that were most seen were in men (codes 2 and 4) with 32 and 24 % respectively and in women in the Private Educational Institution, it was presented in men (37.8%) and in women (39.3%). It was concluded that the prevalence of caries was higher dental in men than in women with a percentage (53.50% and 46.50% respectively), and the age where there is more evidence of carious lesions in 7-year-old schoolchildren with a percentage of (23.16 %) in the National Educational Institutions of the District of Tayabamba, applying the ICDAS II system.
