Preterm delivery is defined as the birth of a baby before 37 completed weeks of gestation. There are several studied mechanisms such as swelling, inflammatory response of the uterus and fetal membranes. In addition, one of the important factors is also maternal age. Objective: To determine the socio-demographic, obstetric and pathological factors associated with preterm delivery in pregnant women of extreme ages at the Hospital Belén de Trujillo. Materials and methods: An observational, analytical, cross-sectional study was carried out. Our population consisted of 170 pregnant women with preterm delivery, grouped into 85 adolescent and 85 elderly pregnant women, in whom sociodemographic, obstetric and pathological factors associated with preterm delivery were evaluated. Statistical analysis was performed using the Chi-square test for qualitative variables. Results: Sociodemographic factors related to preterm delivery in adolescent pregnant women were occupation (p=0.013), with housewife being the most frequent occupation (82.4%). In older pregnant women, incomplete primary education (p=0.045). The pathological factors related to preterm birth in adolescent pregnant women were urinary tract infection (p=0.013) with 67.1% and anaemia (p=0.023) with 37.6%. In older pregnant women, urinary tract infection (p=0.045) accounted for 75.3% and anaemia (p=0.003) and 25.8% of older pregnant women had anaemia. Obstetric factors related to preterm birth in adolescent pregnant women were the inter-gestational period < 18 months (p=0.044) and parity (77.7% of adolescent pregnant women were primiparous). In older pregnant women it was the inter-gestational period >60 months (p=0.005), inadequate prenatal control (p=0.045) and parity 88.8.7% of older pregnant women were multiparous Conclusions: Occupation, urinary tract infection, clinical bacterial vaginosis, anaemia, inter-gestational period and parity are associated with preterm delivery in extremely aged pregnant women in Hospital Belén de Trujillo
