Identify the association between the elevated neutrophil-lymphocyte ratio (RNL) and diabetic nephropathy in patients with type 2 diabetes mellitus. Material and methods: An observational, analytical, cross-sectional study was carried out in a study sample made up of 206 diabetic patients treated at the “Hospital Distrital El Esfuerzo de Florencia de Mora” during the period 2019-2022 who respected the selection criteria. The patient information was collected in a form so that it was later analyzed statistically, using the Chi square and T Student tests. Associations are considered significant if the value of p<0.05 and highly significant if the value of p<0.001. Results: It was found that of the total of 206 (100%) diabetic patients, 114 (55.3%) presented elevated RNL, and among them 77 (37.4%) with diabetic nephropathy and 37 (17.9%) without diabetic nephropathy. At the same time, it was found that 92 (44.7%) did not present elevated RNL, among them 17 (8.3%) with diabetic nephropathy and 75 (36.4%) without diabetic nephropathy. Statistical analysis was performed using the Chi square test where a value of (p=0.000) was found, proving to be highly significant for the association between elevated neutrophil-lymphocyte ratio and diabetic nephropathy. Conclusions: The elevated neutrophil-lymphocyte ratio is a factor associated with diabetic nephropathy in patients with type 2 diabetes mellitus.
