OBJECTIVE: To determine the association between Helicobacter pylori (HP) infection and the development of iron deficiency anemia (IDA) in adult patients at Hospital III José Cayetano Heredia- Piura. MATERIAL AND METHODS: A case-control study was proposed, defining cases as patients diagnosed with Helicobacter pylori according to medical records. Five controls were selected for each case. The diagnosis of iron deficiency anemia was established based on the criteria of the hospital where the patient was treated and determined by the attending physician. Data for this study were collected from medical records. Statistical analysis was performed using Stata v15 software, and Odds Ratios were calculated to quantify the strength of association. RESULTS: The results reveal significant associations between iron deficiency anemia and variables such as sex (p = 0.03) and age (p = 0.04), with a predominance of males (63.16%) and an average age of 48.1 years. Urban origin showed a trend without statistical significance (p = 0.06), with a high percentage of cases in urban areas (89.47%). Helicobacter pylori infection showed a significant association (p = 0.02), with 47.37% of cases infected. Additionally, comorbidities such as type 2 diabetes mellitus, history of gastritis, and history of gastric cancer showed significant associations (p < 0.05). Blood analyses indicated significant differences in various erythrocyte parameters, and the association between H. pylori and iron deficiency anemia was confirmed (p = 0.03), with an OR of 4.51 (95% CI: 2.89-4.78). These results support the relationship between H. pylori and iron deficiency anemia. CONCLUSION: There is an association between Helicobacter pylori infection and the development of iron deficiency anemia in adult patients at Hospital III José Cayetano Heredia- Piura
