The current evidence of risk factors for acute kidney injury in major non cardiac surgeries is extensive and the importance of some of these differs between authors, which makes it difficult to establish a specific prognostic model. The objective of this study was to gather and synthesize the information available on predictors of acute kidney injury, which will allow the early detection of high-risk patients for adequate and timely management, and thereby prevent the appearance of this complication and reduce its incidence. Materials and methods: The search strategy was designed with the help of an experienced researcher, of the studies that compare the variables of interest published in the last 20 years until June 2022, in the databases: Pubmed, Scopus, Web of Science, Cochrane central, LILACS and Dart-Europe. Independently, each investigator assessed the search results and relevant articles were selected. Preoperative and intraoperative data were extracted from patients who underwent major noncardiac surgery. A systematic review and subsequent meta-analysis were performed using the Revman-5.4 software from the Cochrane Foundation. Results: A total of 533 published studies were obtained, of which 10 publications were selected for the analysis. All the studies were cohorts, and included a total of 26,215 patients, of whom 1,776 presented postoperative acute kidney injury, while 24,439 did not. Of the preoperative variables, those that proved to be risk factors for postoperative acute kidney injury were Diabetes Mellitus (OD 1.65, 95% CI 1.20 - 2.27), High blood pressure (OD 1.86, 95% CI 1.36 - 2.54), Treatment with ACEI / ARA II (OD 1.76, 95% CI 1.48 - 2.09), ischemic heart disease (OD 1.93, 95% CI 1.55 - 2.41) and Male gender (OD 1.28, 95% CI 1.07 - 1.53). On the other hand, proved to be protective factors: Female sex (OD 0.72, 95% CI 0.54 - 0.96) and preoperative Hemoglobin (OD -0.41, 95% CI -0.52 - -0.30). The variables Age (OD 2.81, 95% CI -1.81 - 7.42) and preoperative GFR (OD 1.02, 95% CI -9.75 - 11.79) showed a risk trend, and preoperative serum creatinine protective trend (OD 0.02, 95% CI -0.08). - 0.04). The intraoperative variable Operative Time showed a risk trend (OD 23.56, 95% CI -24.23 - 71.36). 7 Conclusions: The presence of comorbidities such as Diabetes Mellitus, High blood pressure and ischemic heart disease, as well as treatment with ACEI / ARB II and male sex significantly increase the risk of developing AKI. Likewise, patients with advanced age and decreased preoperative GFR would tend to be a risk factor for AKI
