Objective: To determine whether spinal anesthesia has fewer negative intraoperative effects than general anesthesia in patients with laparoscopic cholecystectomy. Methodology: An Analytical study was carried out; observational, retrospective cohort type, of 178 patients undergoing laparoscopic cholecystectomy operated under general anesthesia (89) and spinal anesthesia (89). Results: The average intraoperative time was 94.5 min in patients with general anesthesia and 91.5 min in patients with spinal anesthesia. Therefore, patients undergoing general anesthesia increased the risk of having an operative time greater than 60 min with (RR2.01, CI:1.14-2.34), unlike those undergoing spinal anesthesia, which was (p 0.06). Moderate pain was observed with (VAS >3) in the group of patients undergoing general anesthesia (p 0.04), in contrast to patients undergoing regional anesthesia which was (p 0.064). Furthermore, in the immediate postoperative period, as well as a higher risk of nausea and vomiting (p 0.05) (RR 2.13, CI:1.45-2.35) being a risk factor. Conclusion: Patients operated on with general anesthesia had a longer operating time; They presented moderate pain (VAS >3), expressed more symptoms of nausea, as well as greater ventilatory support; Therefore, spinal anesthesia is an effective alternative method to resolve acute cholecystitis in fully selected patients
