Chronic pain is the most frequent and disabling complication in the postoperative period in Liechtenstein hernioplasty, which is why we seek to discover certain factors during the operative technique that decrease pain without influencing the increase in recurrence. Previous studies have been carried out to demonstrate the effectiveness of the use of the adhesive for the fixation of vesus mesh using the conventional technique with suture, however, all of them have different exclusion and inclusion criteria, therefore they have contradictory results. Objective: To compare the effectiveness (postoperative pain and recurrence) of using rubber for polypropylene mesh fixation vs. suture mesh fixation in Liechtenstein inguinal hernioplasty Methods: systematic review of randomized clinical trials and cohort in 4 databases (PubMed, The Web of Science, Scopus and Medline-Ovid) using search strategies. Two authors selected studies, extracted information, and assessed risk of bias independently. The bias analysis was carried out independently by the author and the collaborator, with a third collaborator who entered to dissolve the discrepancies. The tool used to assess the methodological quality of the Randomized Clinical Trials was the Cochrane instrument. Results: Only one multicenter prospective double-blind randomized trial published 4 years ago was included in the review. This study included 370 participants were adults (over 18 years) patients of both genders (332 men and 38 women) with unilateral primary hernias in the study, of these patients randomly received either glue (Histoacryl®) or non-absorbable polypropylene sutures for fixing polypropylene. Postoperative complications, pain, and recurrence were evaluated by an independent blind observer. The result was that the postoperative pain at 8 h, 24 h, 7 days and 30 days was less when glue was used instead of sutures for all measurements (P <0⋅001). The operation was significantly faster using glue ((8⋅7) min versus (11⋅1) min for sutures; P <0⋅001). Analysis of VAS pain scores showed that the use of glue reduced acute postoperative pain during the first month (p <0.001). There were no significant differences between groups in 8 terms of postoperative complications, chronic pain, and early recurrence at follow-up. 1 year (p = 0.467)
