Objective: To analyze whether a high systemic immunity-inflammation index is a predictor of mortality from severe COVID-19. Methodology: A historical cohort study was carried out with the inclusion of patients with severe COVID-19 in the care and temporary isolation center of Bagua between March 2020 and December 2021. Results: There were no significant differences between age and sex, both for the IIIS values and mortality from severe COVID-19. 49.1% of the deceased had lung involvement greater than 75%, and in the survivors it happened in 19.6% (RR: 1.54, p=0.001). Finally, hypoalbuminemia was more frequent in the deceased than in the living (45.6% and 25.5%, respectively), being a risk factor (RR:1.48, p=0.030). It was found that an IIIS greater than 1835 is an independent risk factor for mortality from severe COVID-19 (RRa: 1.726, 95%CI: 1.067-2.792), as well as obesity (RRa: 1.578, 95%CI: 1.06-2.58) and lung involvement (RRa: 1.83, 95% CI: 1.0445-3.205). Conclusions: The IIIS greater than 1835 is a predictor of mortality for patients with severe symptoms of COVID-19.
