To determine the effectiveness of conservative management versus early appendectomy in patients with appendiceal mass. Materials and methods: Five databases were searched, including randomized clinical trials and cohorts comparing early appendectomy versus conservative management in patients with appendiceal mass. The risk of bias was assessed using RoB 2 and ROBBINS-I. The following outcomes were considered: treatment success rate, hospital stay, complications and mortality; meta-analysis of outcomes was performed using Review Manager 5.4, relative risk (RR) was calculated for dichotomous outcomes and mean difference (MD) for continuous outcomes with 95% confidence interval (CI). Results: A total of 1222 articles were reviewed and 9 were selected, with a total of 834 patients with appendiceal mass. Regarding the success rate, there is no statistically significant difference between the two treatments (RR 1.09, 95% CI [0.97 - 1.23], p=0.14, I² 83%). Early appendectomy has shorter hospital stay (MD - 1.53, 95% CI [-2.96 - -0.09], p=0.04, I² 91%) and lower complication rate (RR 0.67, 95% IC [0.45 - 1.00], p=0.05, I² 53%). Mortality was not statistically significant in both groups (RR 0.47, 95% CI [0.09 - 2.50], p=0.38). Conclusions: There was no difference in success rate and mortality between early appendectomy and conservative management. Early appendectomy has a shorter hospital stay and lower complication rate than conservative management.
