The risk of infection in children requiring assistance by mechanical ventilation is high, resulting in Ventilator-Associated Pneumonia (VAP). Care in oral hygiene is an important point to treat and improve to prevent such pathology. For this reason, we studied chlorhexidine, in its different presentations, compared to other mouthwashes, for its useful properties for the prevention of VAP. Objective: To determine if the oral rinse with chlorhexidine is more effective than other oral solutions in the prevention of pneumonia associated with mechanical ventilation in children. Methods: A Systematic Review of randomized clinical trials (RCTs) was performed in 3 databases measuring the efficacy of chlorhexidine rinsing in the prevention of mechanical ventilator-associated pneumonia in children. Two authors selected studies, extracted data, and assessed risk of bias independently. Results: Chlorhexidine does not reduce the risk of pneumonia associated with mechanical ventilation in children compared to placebo (RR 1.14; 95% CI 0.76-1.72; p = 0.50; I2 = 0.0% (0.0%; 46.5%)). Likewise, chlorhexidine not reducing the risk of mortality compared to placebo (RR 0.80; 95% CI 0.52-1.22; p = 0.50; I2 = 0.0%) Finally, regarding the length of hospital stay, there is no significant difference in the difference in means between use of chlorhexidine, compared to placebo (DM 1.01; 95% CI -7.69- 9.71; p = 0.82; I2 = 23.7%). Conclusions: Chlorhexidine oral rinse is not effective in preventing pneumonia associated with mechanical ventilation in children.
