Resumen: Identificar los factores de riesgo para destete fallido en adultos con
ventilación mecánica invasiva
Materiales y Métodos: Se realizó un estudio retrospectivo de casos y controles
realizado en pacientes de 18 años a más ingresados en la unidad de cuidados
intensivos de un hospital de referencia regional durante el periodo 2016 - 2019. Los
casos fueron los pacientes que tuvieron destete fallido de la ventilación mecánica
invasiva y los controles fueron los pacientes con destete exitoso.
Resultados: Se analizaron 35 casos y 70 controles. El alto riesgo nutricional (OR =
2.5; IC 95% = 1,1 - 5,9; p = 0,03), a través del análisis bivariado, fue el único factor
de riesgo presente en la población con destete fallido, por otro lado, la ausencia de
fisioterapia respiratoria se descartó como factor de riesgo para destete fallido en el
análisis bivariado (OR = 0,01; IC 95% = 0 - 0,05; p = 0,00) y multivariado (OR =
0,020; IC 95% = 0,004 - 0,108; p= 0,00).
Conclusiones: Los pacientes en ventilación mecánica invasiva tuvieron mayor
riesgo de destete fallido si presentaban un alto riesgo nutricional mientras que los
pacientes sin fisioterapia respiratoria no presentaron riesgo para destete fallido
To identify risk factors for failed weaning in adults with invasive mechanical ventilation. Materials and Methods: This was a retrospective observational study of case control carried out in patients older than 18 years admitted to the intensive care unit of a regional reference hospital during the period 2016 - 2019. The cases were identified as failed weaning of invasive mechanical ventilator and controls, such as successful weaning. Results: 35 cases and 70 controls were analyzed. The high nutritional risk (OR = 2.5; 95% CI = 1.1 - 5.9; p = 0.03), through the bivariate analysis, was the only risk factor present in the population with failed weaning, on the other hand the absence of respiratory physiotherapy was ruled out as a risk factor for weaning failed in the bivariate analysis (OR = 0,01; IC 95% = 0 - 0,05; p = 0,00) and multivariate (OR = 0,020; IC 95% = 0,004 - 0,108; p= 0,000). Conclusions: Patients in invasive mechanical ventilation had a higher risk of failed weaning if they presented a high nutritional risk while patients who did not receive respiratory physiotherapy did not present a risk for failed weaning
