determine whether stapled hemorrhoidectomy is more effective than transanal hemorrhoidal dearterialization in the treatment of internal hemorrhoids in adults. Material and methods: An update of the systematic review and meta-analysis was carried out, in the screening process allowed to obtain 02 new clinical trials and 09 clinical trials already available from the last review conducted in 2018, to compare the efficacy of stapled hemorrhoidectomy versus transanal hemorrhoidal dearterialization in the treatment of internal hemorrhoids in adults. Results: Of the 11 clinical trials collected, 1164 patients received SH (stapled hemorrhoidectomy) and 586 THD (transanal hemorrhoidal deartherialization); the data obtained for the outcomes, postoperative bleeding, (MD =1.70, 95% CI [1.07 - 2.70] I2=0%, P=0.02); postoperative urinary retention (MD =1. 25, 95% CI [0.66 - 2.34] I2=7%, P=0.50); postoperative pain (MD =0.39, 95% CI [0.19 - 0.58] I2=89%, P=0.0001):hospital stay (MD =0.06, 95% CI [0.00 - 0.11] I2=82%, P=0.05); reoperation rate (MD =1.82, 95% CI [1.02 - 3.23] I2=19%, P=0.04). Conclusions: THD is more effective than SH in reducing postoperative bleeding, postoperative pain and reoperation rate. However, no significant difference was found in the results of hospital stay and urinary retention.
