Objective: To determine if there is a difference in postoperative pain in patients undergoing elective laparoscopic cholecystectomy with general anesthesia vs. spinal anesthesia at the Belén Hospital in Trujillo during the period 2017 - 2021 Material and methods: Observational design, cross-sectional, quantitative approach, basic and comparative type. The population during the study period, of patients undergoing elective laparoscopic cholecystectomy, was 1378 patients (1243 with general anesthesia and 135 with spinal anesthesia), with a sample made up of 270 cases that required general anesthesia and 30 cases with spinal anesthesia. Statistical analysis was using Fisher's exact test. Results: Postoperative pain was greater in females than in males, and the predominant age group was between 30 and 59 years, in patients undergoing general anesthesia vs. Postoperative pain in the first 24 hours affected 17.4% of patients undergoing elective laparoscopic cholecystectomy with general anesthesia, while it occurred in 30.0% of patients with spinal anesthesia. Regarding the type of rescue analgesic used, it can be seen that 8.1% of patients undergoing general anesthesia required opioids, while 9.3% needed NSAIDs. In comparison, patients who received spinal anesthesia, where 20.0% needed opioids and 10.0% NSAIDs. Conclusion: The study establishes that there are no statistically significant differences in postoperative pain in the first 24 hours of patients undergoing elective laparoscopic cholecystectomy with general anesthesia vs. spinal anesthesia, at the Belén Hospital in Trujillo during the period 2017 – 2021
