Resumen: Determinar si en pacientes con colecistitis aguda, el score de Yacoub es
efectivo como predictor de colecistitis aguda gangrenosa en el Hospital Belén de
Trujillo durante el periodo comprendido entre julio del 2019 y junio del 2020.
Materiales y métodos: Se desarrolló un estudio de corte transversal en 110
pacientes con colecistitis aguda que fueron admitidos en el Hospital Belén de
Trujillo, durante junio del 2019 y julio del 2020. Se evaluaron variables clínicas,
quirúrgicas y el score de Yacoub; el test de chi cuadrado (X2
) y T student fueron
utilizadas para en análisis bivariado y la regresión logística para el análisis
multivariado.
Resultados: La prevalencia de gangrena vesicular fue 24.5%; la edad promedio
para los pacientes con y sin colecistitis gangrenosa fueron 52,11 ± 9,38 y 42,51 ±
9,15 (P=0,001); la proporción de varones en dichos grupos fueron 59,26% y
25,30% (p = 0,001). Las variables clínicas hipertensión arterial, diabetes mellitus
tipo 2, obesidad, fiebre, tiempo de enfermedad y el score de Yacoub; así como las
variables quirúrgicas tiempo hasta la cirugía, el score ASA, el grado de colecistitis
y la gangrena vesicular intraoperatoria resultaron estar asociados a la gangrena
vesicular. Se construyó un modelo de predicción compuesta por las variables
fiebre, grado de colecistitis y el score de Yacoub; este modelo resultante tuvo un
nivel de desempeño de predicción del 91,2% comparado con 85% del score de
Yacoub aislado.
Conclusiones: El score de Yacoub tuvo un nivel de desempeño predictor muy
bueno de manera individual para colecistitis gangrenosa, aunque formando parte
de un modelo de predicción logra un desempeño predictor mucho mejor.
To determine whether in patients with acute cholecystitis, the Yacoub score is effective as a predictor of acute gangrenous cholecystitis in Hospital Belén de Trujillo during the period between July 2019 and June 2020. Materials and Methods: A cross-sectional study was developed in 110 patients with acute cholecystitis who were admitted to Hospital Belén de Trujillo, during June 2019 and July 2020. Clinical and surgical variables and the Yacoub score were evaluated; the chi-square test and Student's t-test were used for bivariate analysis and logistic regression for multivariate analysis. Results: The prevalence of vesicular gangrene was 24.5%; the mean age for patients with and without gangrenous cholecystitis was 52.11 ± 9.38 and 42.51 ± 9.15 (P=0.001); the proportion of males in these groups was 59.26% and 25.30% (P=0.001). The clinical variables arterial hypertension, type 2 diabetes mellitus, obesity, fever, time of illness and Yacoub score; as well as the surgical variables time to surgery, ASA score, degree of cholecystitis and intraoperative gallbladder gangrene were found to be associated with gallbladder gangrene. A prediction model composed of the variables fever, grade of cholecystitis and Yacoub score was constructed; this resulting model had a predictive performance level of 91.2% compared to 85% of the Yacoub score alone. Conclusions: The Yacoub score had a very good level of predictive performance individually for gangrenous cholecystitis, although as part of a prediction model it achieves a much better predictive performance.
