Resumen: Demostrar que la hiponatremia es un factor de riesgo para encefalopatía
hepática (EH) en pacientes con cirrosis del Hospital Regional Docente de Trujillo
durante el periodo enero 2018 – diciembre 2022.
Metodología: Estudio de casos y controles que incluyó a 202 pacientes con CH,
siendo agrupados en 101 casos con EH y 101 controles sin EH. Se obtuvo el sodio
sérico del primer análisis de gases arteriales tomados en el ingreso del paciente a
emergencia y se valoró la EH por medio de la escala de West Haven de cada
historia clínica. Se calculó el odds ratio para cada variable y se realizó regresión
logística para la evaluación de factores intervinientes.
Resultados: La frecuencia de la hiponatremia en los pacientes con EH fue del
62.4%, mayor que en los controles 17.8% (p<0.001), siendo un factor de riesgo
significativo (ORa: 8.64, p<0.001); además, se determinó que el valor de sodio tiene
una correlación negativa con el grado de EH (Rho: -0.550, p<0.001). El estadio
Child Pugh C (ORa: 2.16, p<0.037), la constipación (ORa: 3.06, p<0.018), la
hemorragia digestiva alta (ORa: 3.18, p<0.003) y la peritonitis bacteriana
espontánea (OR:a 3.19, p<0.029) demostraron ser factores de riesgo
independientes para EH.
Conclusiones: La hiponatremia es un factor de riesgo asociado a EH en pacientes
con CH.
To demostrate that hyponatremia is a risk factor for hepatic encephalopathy (HE) in patients with liver cirrhosis (LC) at the Trujillo Regional Teaching Hospital during the period January 2018 – December 2022. Methodology: Case-control study that included 202 patients with LC, grouped into 101 cases with HE and 101 controls without HE. Serum sodium was obtained from the first arterial blood gas analysis taken at the patient's admission to the emergency room, and HE was assessed using the West Haven scale from each medical record. The odds ratio was calculated for each variable and logistic regression was performed to evaluate intervening factors. Results: The frequency of hyponatremia in patients with HE was 62.4%, higher than in controls 17.8% (p<0.001), being a significant risk factor (aOR: 8.64, p<0.001); Furthermore, it was determined that the sodium value has a negative correlation with the degree of HE (Rho: -0.550, p<0.001). Child Pugh C stage (aOR: 2.16, p<0.037), constipation (aOR: 3.06, p<0.018), upper gastrointestinal bleeding (aOR: 3.18, p<0.003) and spontaneous bacterial peritonitis (aOR: 3.19, p<0.029) demonstrated to be independent risk factors for HD.
