Objective: To demonstrate that the prophylactic use of aspirin of 150 mg prevents the onset of early onset preeclampsia in pregnant women with risk factors treated in the Department of Gynecology and Obstetrics of the Hospital Santa Rosa II-2, in the period 2017-2019. Methodology: Quantitative, observational, retrospective study, unpaired cases and controls. 142 pregnant women were included that met the inclusion and exclusion criteria validated for this research work. The computer programs: IBM SPSS v.23 and Microsoft Excel 2016, were used to analyze the data, and the subsequent tabulation of them. To establish the relationship between variables, the Chi-square test with a significance level of 5% was used to subsequently analyze the risk through the Odds ratio. Results: As main results it was obtained that the number of years studied is a sociodemographic risk trigger to have as a consequence preeclampsia of early onset (p = 0.018); (OR: 2.42; 95% CI 1.12-5.26), in addition, as a clinical risk trigger, there was a history of preeclampsia (p <0.01); (OR = 7.12; 95% CI 3.26-15.54), history of hypertension (p <0.01); (OR = 5.01; 95% CI 2.27-11.09) and primigestation (p = 0.026); (OR = 2.12; 95% CI 1.05-4.29). Likewise, it was evidenced that there is a relationship between the prophylactic use of aspirin and the development of early onset preeclampsia, with protective factors being prophylactic use in a period <= 12 weeks (p <0.01); (OR = 0.32; 95% CI 0.15 - 0.71) and strictly comply with the assigned treatment (p = 0.028); (OR = 0.32; 95% CI 0.15 - 0.71). Conclusions: There are sociodemographic factors such as years of instruction, clinical as a previous picture of preeclampsia, hypertension and primigestation associated with early onset preeclampsia, also, the prophylactic use of aspirin 150 mg at a period less than twelve weeks and a strict treatment result Be protective factors.
