Objective: To evaluate the association between the Alvarado score and the macroscopic aspect of the appendix in appendectomized patients at the Hospital de la Amistad Perú Corea Santa Rosa II 2 - Piura during the period January - December 2018. Methods: A study was conducted on 247 patients with a diagnosis of appendicitis who were operated on. The Alvarado score was applied to them and the surgical report was reviewed to identify the macroscopy of the appendix, the results were filled out on a data collection form, and they were processed in STATA v.15. Results: Laboratory findings such as neutrophilia 220(89.07%) and leukocytosis 210(85.02%) were found first, followed by specific clinical signs such as localized pain in the lower right quadrant 205(83%) and positive Blumberg sign 188(76.11%). In the macroscopic findings, the phlegmy or fibrous phase was found to be the most prevalent 118 (47.77%). A significant association was found p<0.001, between the categories obtained by the score in the Alvarado score and the macroscopic stage of the appendix. The 7-point cut-off point had 100% sensitivity and 51.94% specificity for complicated appendicitis. Conclusion: An association was found between high scores on the Alvarado Score and the macroscopic stage of the appendix.
