To assess whether pregnant women with cesarean delivery in their second stage of labor have risk of preterm delivery than pregnant women with vaginal delivery in their subsequent pregnancy. Methods: A retrospective cohort study was carried out, evaluating 614 clinical records of pregnant women who had been treated during the period from 2013 to 2017, two study groups were formed: cohort I = 307 pregnant women with a history of cesarean section in their second stage of delivery and cohort II = 307 pregnant women who had vaginal delivery, all of them attended in the Obstetrics service of the Teaching Regional Hospital of Trujillo. Results: The average maternal age in cohort I was 24.74 ± 4.60 years and in cohort II 25.53 ± 4.44 years (p <0.05); The number of prenatal controls also showed a significant difference between the two cohorts, 7.20 ± 2.91 and 6.61 ± 3.21 for cohorts I and II respectively (p <0.05). The degree of education, the marital status, the pre gestational body mass index and the gestational age at delivery did not result in significant differences. An association was found between the antecedent of cesarean section in the second stage of labor and the presence of preterm delivery in the subsequent pregnancy, with an incidence of preterm labor in cohort I and II of 6.51% and 2.93. % respectively (p <0.05); with a RR = = 1.41 IC 95%: 1.09 - 1.82. Conclusions: This study shows that cesarean section in the second stage of labor is a risk factor for preterm delivery in the subsequent pregnancy.
