Objective: To determine the prognostic value of mortality of urea nitrogen and creatinine on admission in patients with moderate and severe thermal burn injuries. Material and methods: Observational study, with a retrospective cohort design made up of patients with moderate and severe thermal burns treated in the two hospitals of Piura during the period 2010 to 2023. The variables were summarized, making comparisons of proportions and medians. The analysis of ROC curves was performed with a report of the area under the curve and the report of the Relative Risk magnitude estimator. The significance value was established with a p less than 0.05. Results: A significant difference in the frequency of mortality was found in the exposed cohort (creatinine and/or elevated urea nitrogen on admission) with a value of 87.5%. Belonging to the exposed cohort obtained a good ability to predict death registration (AUC 0.88, 95% CI 0.81 – 0.97). For each point increase in creatinine and urea nitrogen, the risk of death increased by 4.3 (RR: 4.32, 95% CI 2.51 – 7.44) and 6% (RR: 1.06, 95% CI 1.04 – 1.08) times more. The cut-off points of creatinine (≥ 2) and urea nitrogen (≥ 27) obtained a 94% probability of predicting death registration (AUC: 0.94, 95% CI 0.85 – 0.99), with acceptable levels of sensitivity and specificity. Conclusions: Blood creatinine and BUN values are prognostic factors for mortality in patients with moderate and severe burns
