Objective: To determine if there is an association between perioperative factors and postoperative complications of emergency surgery for incarcerated inguinal hernia. Materials and method: A retrospective, observational, analytical case-control study was carried out with 105 patients undergoing emergency surgery for incarcerated inguinal hernia. They were divided into 35 cases (postoperative complications) and 70 controls (no complications). The Chi Square test (p < 0.05) was used to evaluate associations between variables. Results: It was shown that, in the present study, the postoperative complications were: hernia recurrence (60%), seroma (40%), surgical site infection (28.6%) and mortality (0%) of the patients who presented minus one complication. Furthermore, it was evident that a hernia sac diameter ≥ 10 cm was the only statistically significant perioperative factor associated with postoperative complications (OR: 3.16, CI: 95%, p=0.045), with a frequency of 22.9% in this group. Duration of hernia incarceration, operation time, repair technique, postoperative stay, type of anesthesia, location of hernia, age, concomitant disease, and history of abdominal wall hernia showed no association with postoperative complications (p=NS). Conclusions: The present study determined that a hernia sac size ≥10 cm is the only perioperative factor associated with postoperative complications in emergency surgery for incarcerated inguinal hernia. Hernia recurrence is the most common complication, followed by seroma and surgical site infection
