Objective: Determine the effectiveness of early vs late laparoscopic cholecystectomy for prevention of recurrent biliary pancreatitis. Material and Methods: A systematic review and meta-analysis was carried out, selecting articles from the database of Pubmed/Medline, Scopus, Web-of Science and the virtual health library, using search strategies with controlled terms, original studies were chosen. primaries that meet our PICO question. The study selection process was graphed using a Prisma flowchart. The relevant data were extracted into descriptive tables using the Excel program; The risk of bias and recurrence of pancreatitis were evaluated as an outcome. Results: A total of 5 primary studies were entered. The prevalence of recurrence of acute biliary pancreatitis after early cholecystectomy was lower than that found after late laparoscopic cholecystectomy; fluctuating between 0- 5% with the early technique; and between 9-44% with the late one. The pooled relative risk for the prevalence of recurrent biliary pancreatitis was 66% lower in patients who underwent early laparoscopic cholecystectomy. In studies that included only patients with mild acute pancreatitis at admission to PCOS and who underwent early cholecystectomy, they had a lower risk of pancreatitis recurrence compared to those that included mild-severe pancreatitis. Conclusion: Early laparoscopic cholecystectomy is associated with a lower prevalence of recurrent biliary pancreatitis; however, this result can only be recommended in those patients who underwent surgery with mild acute biliary pancreatitis
