Objective: Identify the factors associated with the need for neonatal resuscitation in newborns with meconium-stained amniotic fluid from the neonatology service of the Hospital Belén de Trujillo during January 2016 to December 2022. Materials and methods: Observational, analytical, retrospective, case-control study. Data were collected from the medical records of newborns and mothers from the neonatology service of the Hospital Belén de Trujillo in the period 2016 to 2022, which met the inclusion and exclusion criteria, obtaining a total of 252 records, being 84 cases and 168 controls. The risk factors associated with the need for neonatal resuscitation were studied and bivariate logistic regression analysis and Odds Ratio (OR) calculation were applied. Result: Of the 252 newborns with meconium-stained amniotic fluid, 84 required neonatal resuscitation. The bivariate logistic regression showed that, among the factors studied, the presence of thick meconium fluid (OR= 11.57; IC 95%: 4.34-30.85, p: 0,000), post-term gestation (OR= 4.89; IC95%: 1.12-21.41, p: 0,035), and premature detachment of placenta (OR= 20.77; IC95%: 2,72-158,91; p: 003) are factors that significantly increase the need for neonatal resuscitation for a newborn with meconium-stained amniotic fluid. Likewise, it was found that the risk factor that predominated with 86.9% was thick meconium fluid. Conclusion: Risk factors associated with the need for neonatal resuscitation in newborns with meconium-stained amniotic fluid are thick meconium fluid, post-term gestation, and placental abruption
