OBJECTIVE: To determine if red blood cell distribution width is a prognostic factor for mortality in patients with severe COVID-19 treated at Hospital III-1 José Cayetano Heredia Piura. MATERIAL AND METHODS: The study design was observational, longitudinal, and retrospective cohort. The research was conducted on 122 patients diagnosed with COVID-19 treated at Hospital III-1 José Cayetano Heredia Piura during the period 2020- 2021. Clinical records were analyzed through simple random sampling of patients who met the selection criteria. Chi-square test was used with a significance level of 5% to establish the relationship between qualitative variables, and the Student's t-test was used for quantitative variables. Relative risk (RR) was used as a statistic. RESULTS: The study revealed significant differences in red blood cell distribution associated with severe COVID-19. Statistically relevant variations were found in gender (p=0.05) and age (p=0.03) related to red blood cell distribution. Additionally, a significant association was observed between elevated red blood cell distribution and mortality (p=0.03). Significant associations were also identified with hypertension (p=0.04) and obesity (p=0.03), while the Charlson index showed a trend (p=0.06). Relationships between red blood cell distribution and age >65 years (p=0.02) and disease duration >9 days (p=0.04) were also significant. Although there were no associations with hospitalization duration >15 days in mortality (p>0.05), these findings highlight the prognostic role of red blood cell distribution in patients with severe COVID-19. CONCLUSION: Elevated red blood cell distribution is significantly associated with an increased risk of mortality in patients hospitalized with severe COVID-19, emphasizing its relevance as a prognostic factor in this population
