Objective: To determine if the heart rate at admission is a factor associated with in-hospital mortality in patients with acute coronary syndrome. Material and methods: A retrospective, analytical, cross-sectional study was carried out in which 171 patients with acute coronary syndrome were included, from the Victor Lazarte Echegaray hospital, during the 2014-2018 period, which met the selection criteria which were divided into 2 groups : 35 deceased patients and 136 survivors; applying the odds ratio, and the chi-square statistical test. Results: The average age and frequency of diabetes mellitus, arterial hypertension and functional class Killip III and IV were significantly higher in the group of deaths than in the group of survivors (p <0.05). Tachycardia is a factor associated with in-hospital mortality in patients with acute coronary syndrome with an odds ratio of 4.1 which was significant (p <0.05). Bradycardia is a factor associated with in-hospital mortality in patients with acute coronary syndrome with an odds ratio of 4.1 which was significant (p <0.05). In the multivariate analysis, tachycardia, bradycardia, diabetes mellitus, arterial hypertension and Killip III and IV functional class are recognized as factors associated with mortality in patients with acute coronary syndrome (p <0.05). Conclusion: The heart rate at admission is a factor associated with in-hospital mortality in patients with acute coronary syndrome.
