Objective: To determine which characteristics influence the prolonged post-operative stay in patients undergoing conventional versus laparoscopic appendectomy for complicated appendicitis in Hospital III-1 ESSALUD José Cayetano Heredia Piura 2018. Methodology: Analytical, observational, cross-sectional and correlational study. The sample consisted of 163 patients from a population of 283 cases of complicated appendicitis. The sample was calculated using the formula for known populations using the Epi info statistical program, resulting in a total of 163 patients; of which 109 cases of complicated acute appendicitis were subjected to conventional appendectomy and 54 to laparoscopic appendectomy. We retrospectively analyzed the variables of age, sex, weight, time of illness, hospital stay, leukocytosis, appendix position and postoperative complications for each type of surgery. Results: When carrying out the bivariate analysis with respect to open surgery, it was found that the time of illness> 12 hours, leukocytosis, drain placement, residual abscess and prolonged pain are associated with prolonged stay in the patients submitted to this type of intervention. In the same way, leukocytosis, drain placement, residual abscess, intestinal obstruction and prolonged pain increase the prevalence rate of prolonged stay in 3.74 (p = 0.01, RPa [1.34 - 10.41]), 2.2 (p <0.01, RPa 2.2 [1.71 - 2.83]), 5.36 (p <0.01, RPa [3.81 - 7.55]) and 2.1 (p <0.01, RPa [1.75 - 2.5]) times respectively. With respect to patients undergoing laparoscopic surgery, drain placement was associated with prolonged hospital stay. Likewise, it was found that leukocytosis increases by 2.35 (p <0.01, RPa 2.35 IC [1.83 - 3.01]) times the prevalence ratio of prolonged stay, drain placement increases by 2.75 (p = 0.02, RPa 2.75 IC [1.13 - 6.69]) times the prevalence ratio of presenting prolonged stay in this type of intervention. Conclusion: The hospital stay of laparoscopic appendectomy and conventional appendectomy could increase depending on the clinical characteristics of each patient in the postoperative period.
