To determine if the distribution width of red blood cells as a non-invasive biomarker is useful for the diagnosis of acute appendicitis in pediatric patients. Materials and Methods: A study of diagnostic tests was carried out, in which 195 medical records of pediatric patients under 15 years of age with suspected acute appendicitis undergoing surgery at the Virgen de la Puerta High Complexity Hospital were analyzed during the period between January 2019. - December 2023. According to the pathological report, the patients were classified as patients with acute appendicitis and without appendicitis. Sociodemographic and laboratory data such as leukocyte count, C-reactive protein (CRP), red blood cell distribution width (RDW) and mean platelet volume (MPV) were compared. Results: Of the 195 pediatric patients included in the study, 121 were men (68.8%) and 55 were women (31.3%). The median age value was 12 (IQR: 11-13). The white blood cell count, CRP, and MPV values of patients with appendicitis were higher than those of patients without appendicitis, while the RDW in the group of patients with acute appendicitis was lower compared with the group without acute appendicitis. However, acute appendicitis was not associated (p > 0.05), except for CRP and RDW (p < 0.001), which did present statistical significance. Using the cut- off point of ≥ 13, the diagnostic validity of the RDW had a sensitivity of 36.26% and specificity of 84.21%, positive predictive value 95.52% and negative predictive value 12.50%. Subsequently, when performing the ROC curve, the area under the ROC curve was 0.572 (95% CI = 0.483- 0.66; p > 0.05). Conclusions: RDW is not a useful biomarker for detecting acute appendicitis in pediatric patients.
