Objective: To determine the relationship between the Neutrophil-to-lymphocyte ratio (NLR) of complicated acute appendicitis in Hospital III Cayetano Heredia during 2018. Methods: Analytical, observational, retrospective, transversal and correlational study. The sample consisted of 150 clinical histories of patients with a diagnosis of complicated acute appendicitis who met the inclusion and exclusion criteria. The information collected was digitized in a database in Excel 2010, and then transferred to STATA 14, software in which statistical analysis, as well as sensitivity and specificity were performed. Results: A total of 150 patients with acute appendicitis were studied, of which 68.7% presented complicated acute appendicitis. A higher prevalence of men was found, with an average age of 34,7 years. There was an association between INL and the development of acute appendicitis (RPa=2,13; 95%IC: 1,20 – 3,05; p<0,001). Area analysis under the curve yielded that the cut-off point with better sensitivity (83,33%, 95%IC: 73,58 – 93,08) and specificity (100%, 95%IC: 98,33 - 100) was the INL value would be 4,65. Conclusions: A NLR greater than 4,65 was associated with a sensitivity of 83,3% and a specificity of 100% to identify patients with complicated acute appendicitis.
