Objective: To determine the association between pregestational BMI and alterations in fetal-neonatal development and growth. Materials and methods: Retrospective cohort study. The data was obtained from the Perinatal Information System (SIP 2000). The total population studied was 183926 of the hospitals: Hospital Nacional Docente Madre Niño San Bartolomé (HONADOMANISB) and Hospital Nacional Dos de Mayo (HNDM). A census was taken of the pregnant women of two national hospitals in Lima-Peru who gave birth during the period of 2000-2015. The independent variable was pregestational BMI and the dependent variables were Intrauterine Growth Restriction (IUGR) and prematurity. Results: BMI underweight, overweight, pregestational grade 1 obesity accounted for 2.64% (2786), 27.79% (29297) and 6.88% (7247), respectively. The IUGR was presented in 0.66% (691) of the newborns. Prematurity represented 1.75% (1944). The low pre-pregnancy weight has 2.27 times the risk of presenting IUGR (RR 2.27 IC95%: 1.69-3.05). Pre-pregnancy overweight has a 30% lower risk of presenting IUGR (RR: 0.70 IC95%: 0.58-0.84). Pregestational grade 1 obesity has a 38% lower risk of developing IUGR (RR: 0.62 IC95%: 0.43-0.89). Low pre-pregnancy weight showed 1.28 times more risk of prematurity (RR: 1.28 IC95%: 0.99-1.65). The risk of prematurity was 1.15 times higher in pregnant women with obesity grade 2 and 3 pre-pregnancy (RR: 1.15 IC95%: 0.82-1.59). Conclusions: low pre-pregnancy weight was a risk factor for IUGR. Low weight, obesity grade 2 and 3 pregestational showed weak association, not significant to prematurity.
