Introduction: HIV/AIDS is a public health problem that affects the lives of 39.87 million people in the world. Initially, the epidemic affected mainly men, but according to current data, 57% of those infected with HIV/AIDS are women. 35% of patients do not adhere to antiretroviral therapy due to adverse events and 50% of chronic patients comply with their treatment; poor adherence is responsible for between 5 and 10% of hospital admissions, 2.5 million medical emergencies and 650,000 deaths per year. The PROCET program of the Regional Hospital has 615 patients with HIV who receive antiretroviral therapy, of which more than 40% do not adhere to the therapy, due to the adverse events they produce. Objective: To determine the relationship between adverse events and antiretroviral therapeutic adherence, in patients with HIV treated at the Regional Hospital of Cusco, January - December 2021-2022. Materials and Methods: The study design was retrospective cohort. Sampling was non- probabilistic and for convenience. For the statistical analysis, the calculation of the RR and chi square was used and for the adjustment a multivariate logistic regression was carried out with the variables: adverse events, therapeutic adherence, age, sex and level of instruction. 615 clinical records were reviewed, our sample for bias control being 296 patients meeting the inclusion criteria. Results: There is a relationship between adverse events and therapeutic adherence with RR: 2,053; 95% CI (1,332-3,165); p=0.003, with gastrointestinal events being the most relevant with 29.4% within them nausea and abdominal pain, followed by psychiatric events with 21.2% with depression, the drug Efavirenz (23.8%) together with zidovudine (23%) they are responsible for producing greater adverse events; patients do not adhere to treatment in 76.4%. Conclusions: It was demonstrated that, if there is a relationship between adverse events and antiretroviral therapeutic adherence, in patients with HIV treated at the Regional Hospital of Cusco, January - December 2021-2022.
