Endocranial hypertension is one of the most common causes of death of patients with severe head injury and decompressive craniectomy is part of the included treatment, although it is not yet widely accepted for its benefit not only in the short but also in the long term. Objectives: To determine the epidemiological, clinical and tomographic characteristics of patients with traumatic endocranial hypertension undergoing decompressive craniectomy at Hospital Antonio Lorena in the period 2015 to 2019. Methods: A descriptive, cross-sectional and retrospective study was conducted. The sample was 67 patients, which constituted the overall number of cases for the referred period. For the analysis, the statistical package SPSS 24 was used, using figures and frequency tables. Results: Among the epidemiological characteristics of the patients, it was found that 65.7% were male, they mostly come from marginal urban areas (38.8%), and the most observed occupations were farmers, students, housewives, and merchants. Among the clinical characteristics, the trauma mechanism was 47% of the patients had a trauma caused by falls, 55.2% of the cases were admitted to surgery between 12 and 24 hours after the trauma, 61.2% presented a scale of Glasgow at admission between 7 and 8, presenting mostly isochoric (53.7%) and reactive pupils (88.1%), in addition to the motor neurological deficit, 64.2% presented hemiparesis and the babinski sign was present in 56.7% of the patients. Meanwhile, in relation to tomographic characteristics, 50.7% of the patients had a Marshall scale corresponding to a diffuse IV lesion. Conclusions: Male patients are the most affected by traumatic endocranial hypertension, having agriculture as their main occupation and were from the marginal urban area. The greatest percentage of patients with severe TBI presented falls as a mechanism of trauma, a Glasgow scale on admission between 7-8, with isochoric and reactive pupils, with the presence of hemiparesis and babinski sign, undergoing decompressive craniectomy among the 12-24hr. According to the Marshall tomographic classification, there was a higher percentage of diffuse IV injury.
