To determine whether there is an association between prenatal exposure to tobacco smoke and delayed psychomotor development in children under 5 years of age treated in the CRED area of the José Cayetano Heredia Regional Hospital in Piura during the 2023-2024 period. Material and Method: This case-control thesis utilized mixed data collection through medical records of children under 5 years old attending the Growth and Development Program at Hospital Regional José Cayetano Heredia of Piura in the period 2023 to 2024. EEDP and TEPSI scales were used to assess psychomotor development. Descriptive statistical tables were employed, and a p- value ≤ 0.05 was considered significant. Results: The study shows that delayed psychomotor development in children under 5 years is significantly associated with prenatal exposure to tobacco smoke, with boys being more affected (P=0.04). Children with delays had lower Apgar scores at birth, and their caregivers tended to have a lower educational level. Additionally, these children were more likely to live in urban areas and be exposed to tobacco smoke, either directly or passively (P=0.03). The educational level also influenced the children's development, suggesting that the family and social environment plays a crucial role in their psychomotor evolution. Conclusion: If there is an association between prenatal exposure to tobacco smoke and delayed psychomotor development in children under 5 years of age treated in the CRED area of the José Cayetano Heredia Regional Hospital in Piura.
