This study explores and describes the affective gestural context that accompanies moments of mentalization in the interaction between patient and therapist in a case of focal psychotherapy with a young adult with depressive symptoms. Using a qualitative approach and a single-case design, the research analyzes non- verbal interactions through FaceReader, focusing on mentalization fragments from the middle phase of an Interpersonal Dynamic Therapy (IDT) process. The results suggest that incorporating gestural elements in IDT significantly enriches the understanding of therapeutic dynamics, laying the groundwork for future research. Specifically, the study indicates that emotional attunement could be crucial for facilitating selfregulation and integrating dissociated affects in the patient with depressive symptoms. Additionally, it suggests a relationship between the "not knowing" attitude and the "surprised" affect, offering a new perspective on countertransference and the therapist's emotional management. The research seeks to continue contributing to the understanding of non-verbal communication in therapy, emphasizing its relevance for clinical practice and therapist training.
