Determine if hypertension is a risk factor for biochemical recurrence after radical prostatectomy. MATERIAL AND METHODS: A case-control study was used, which consisted in the collection of medical records of all patients from ““IREN - Norte““ and ““HVLE““, with a diagnosis of organ-confined prostate cancer, who underwent radical prostatectomy during the years from 2009 to June 2022. The results were processed using the ““IBM SPSS Statistics 26 Statistical Package““. RESULTS: A total of 190 prostatectomized patients were obtained, of which 69 presented biochemical recurrence (cases) and 121 did not (controls). A higher frequency of hypertension was determined in the case group (50.7%) than in the control group (33.1%). In turn, it was determined that arterial hypertension is a risk factor for biochemical recurrence after radical prostatectomy. (OR= 2.08, p=0.017). After analyzing the intervening variables, a significant statistical association was found only with the variable status of the positive surgical margin. (p=0.003). Finally, the multivariate analysis determined that hypertension adjusted for the status of the positive surgical margin continues to be a risk factor for biochemical recurrence. (p=0.021) CONCLUSIONS: Hypertension is a risk factor for biochemical recurrence after radical prostatectomy
