Objective: We set out to demonstrate that twin gestation is a risk factor for the development of premature rupture of membranes. Material and Methods: A retrospective, cohort study was carried out. The study population consisted of 158 pregnant women; which were divided into 2 groups: with or without twin gestation; that met the inclusion criteria, the relative risk and the chi square test were calculated. Results: The average ages of pregnant women with and without twin gestation were 28.5 + - (7.1) years and 27.7 + - (6.5) years respectively, with p = 0.068, in terms of parity 1.6 + - (1.2) children and 1.4 + - (0.9) children p = 0.076 in each group, noting that the origin in the group with twin gestation was 90% urban and 10% rural while the group without twin gestation was 94% urban and 6% rural; with a p 0.071, no significant differences were observed in relation to the variables maternal age, parity or provenance among pregnant women with or without premature rupture of membranes. The incidence of premature rupture of membranes in patients with twin gestation was 25% and in patients without twin gestation was 11%, in addition the degree of association that implies the presence of twin gestation for coexistence with premature rupture of membranes was evaluated; which is expressed as a relative risk of 2.2 p = 0.039, which was significant (p <0.05). Conclusions: The twin gestation is a risk factor for premature rupture of membranes
