Recently, the use of proton pump inhibitors (PPIs) has been associated with an increased risk of pneumonia. We aimed to confirm this association. Material and Methods: We conducted a case-control study with patients treated at the Department of Medicine Hospital Victor Lazarte Echegaray. Cases (n = 62) are defined as all patients with a first diagnosis of community-acquired pneumonia during 2012 to 2014. We selected 124 control subjects who were matched to cases by age and sex. Data on the use of proton pump inhibitors (PPIs) were extracted from the medical records of the patients studied. We look also at the pharmacy system, emergency and outpatient system. Results: The odds ratio (OR) was found in the association of PPI use with community-acquired pneumonia was 1.9 (95% confidence interval: 1.03 to 3.57). The recent initiation of PPI treatment (30 days before the index date) showed a strong association with community-acquired pneumonia (OR: 2.46; 95% CI: 1.02 to 5.89), whereas no association was found with treatment that started a long time ago (OR: 1.64; 95% CI 0.68 to 3.94). Conclusion: The use of PPIs, especially when recently begun, is associated with an increased risk of community acquired pneumonia.
