To determine whether ultrasound appendiceal outer diameter of 7 mm has higher diagnostic yield than the value of 6 mm in children with acute appendicitis. MATERIAL AND METHODS: We performed an observational, analytical study of diagnostic tests, which evaluated 68 children who underwent surgery for acute appendicitis in the study period between January 2010 to December 2013. RESULTS: The mean age of patients with acute appendicitis (AA) was 12.51 ± 2.98 years and in those without AA was 13.45 ± 2.58 years, the proportion of male patients in patients with and without AA was 57, 89 % and 36.36% respectively. Regarding the Outer appendiceal diameter (OAD) of Appendix seen by ultrasound, we observed that the overall rate of patients with preoperative diagnosis of AA was 7.64 ± 2.33 mm, compared to the values in the OAD patients with and without AA were 7.97 ± 2.38 and 5.89 ± 0.87 mm . By categorizing the OAD with a cut of 6 mm was found that 77.19 % of patients had a OAD AA ≥ 6 mm and in which not only the AA had 54.55 %, and the OAD categorized with a cutoff of 7 mm was found that 68.42% of patients had a MOD AA ≥ 7 mm and in which there were only 18.18 % AA . When calculating the area under the curve (through the ROC curve ) of the OAD with a cutoff of 6 mm and the OAD with a cutoff of 7 mm, the following values were taken : OAD6 = 0.613 and OAD7 = 0.751. CONCLUSIONS: The ultrasonographic outer appendiceal diameter of 7 mm has higher diagnostic yield than the value of 6 mm in children with acute appendicitis.
