Objective: The objective of this research is to identify the clinical factors associated with pathological Doppler of uterine arteries in pregnant women at the José Cayetano Heredia-Piura Hospital. Material and Methods: This study is a retrospective, analytical, cross-sectional observational study composed of 100 pregnant women who underwent uterine artery Doppler in the first trimester during the year 2023-2024 at Hospital III-1 José Cayetano Heredia-Piura. Clinical data were collected from the medical records of the patients who met the inclusion criteria. Statistical analysis included multivariate logistic regression to determine odds ratios (OR) and 95% confidence intervals (CI) for several clinical factors. Results: Several clinical factors were significantly associated with pathological Doppler of the uterine arteries. The study found that a history of preeclampsia increased the odds of a pathological Doppler by 4.4 times (95% CI: 1.6-11.8, p=0.003). Chronic hypertension was associated with a 9.24-fold increase in the likelihood of pathological Doppler (95% CI: 1.04-82.4, p=0.028). Pre-gestational obesity was a significant predictor with an OR of 6.57 (95% CI: 1.93-22.3, p=0.001), and pregestational diabetes was associated with a 9.3-fold increase (95% CI: 1.8-45.8, p=0.002). Advanced maternal age (≥35 years) also showed a strong association with pathological Doppler (OR: 6.65, 95% CI: 2.15-20.5, p=0.001). Conclusions: The previous preeclampsia, chronic hypertension, pregestational obesity, history of diabetes and pregestational diabetes are factors associated with pathological uterine artery Doppler. Early identification of these patients by uterine artery Doppler ultrasound may allow timely interventions and improve obstetric outcomes
