The objective of the study was to determine the accuracy of the femoral cortico-diaphyseal radiographic index for the assessment of osteoporosis in patients of the Hospital de Alta Complejidad Virgen de la Puerta La Libertad assistence network. In order to establish a simple and easy-to-use tool in clinical practice. Likewise, to establish if there is a relationship between osteoporosis and other comorbidities such as hypertension, obesity, dyslipidemia and diabetes mellitus. Materials and methods: Clinical records of patients treated in the Service of Radiology of the Virgen de Puerta de Trujillo High Complexity Hospital in the period 2017 - 2021 were examined. Patients who met the selection criteria were included until reaching the sample size in a manner random; Subsequently, the value of the cortex-diaphyseal radiographic index was recorded, to later contrast the diagnosis of osteoporosis by means of bone densitometry Results: The study included a total of 132 patients who underwent proximal radius radiography. The proximal cortex-diaphyseal radiographic index in the assessment of osteoporosis showed acceptablr accuracy in predicting osteoporosis, the cut-off value was 0.2950, with a sensitivity of 40% (95% CI 24.58-55.42) and a specificity of 88.51% (95% CI). 81.23-95.78); The positive predictive value was 64.29% (95% CI 44.75-83.82) and the negative predictive value was 74.04% (95% CI 65.13-82.95); The positive likelihood ratio was 3.48 (95% CI 1.76-6.9) and a negative likelihood ratio was 0.68 (95% CI 0.53-0.87). The area under the curve (AUC) was 0.686 (95% CI 0.590-0.782). Conclusions: The femoral cortex-diaphyseal radiographic index presents a moderate accuracy for the diagnosis of osteoporosis in patients of the Hospital de Alta Complejidad Virgen de la Puerta de Trujillo.
