To identify the neutrophil/lymphocyte ratio as a predictor of mortality and complications in patients diagnosed with acute cholecystitis at the Trujillo Regional Teaching Hospital - Peru, from 2019-2021. Material and methods: An analytical, retrospective cohort study was carried out in which 172 patients with acute cholecystitis were included, according to selection criteria which were divided according to the value of the elevated or not elevated neutrophil-lymphocyte index, it was calculated the chi square and the relative risk statistic. Results: No significant differences were observed in the average age or in the frequency of sex, diabetes mellitus type 2 or arterial hypertension between patients with a high or low neutrophil-lymphocyte ratio (p>0.05), the high neutrophil-lymphocyte ratio is not a predictor of mortality in patients diagnosed with acute cholecystitis, with a relative risk of 3, which was not significant (p>0.05), the high neutrophil-lymphocyte ratio is a predictor of complications in patients diagnosed with acute cholecystitis, with a relative risk of 1.27, which was significant (p<0.05). Conclusion: The neutrophil/lymphocyte index is a predictor of complications in patients diagnosed with acute cholecystitis at the Trujillo Regional Teaching Hospital - Peru, from 2019- 2021.
