The objective of this study was to determine if anemia is a factor associated with preterm birth in pregnant women with extremely old ages, treated at the Support Hospital II Sullana during 2016-2020. An analytical, observational and retrospective research was carried out, with a case-control design. The sample included 93 cases and 93 controls, collecting data through a form designed for this purpose. The results indicated that 65.6% of the cases and 66.7% of the controls corresponded to pregnant women between 36 and 39 years old. Regarding the educational level, 39.8% of the cases were in primary school and 48.4% in secondary school, a similar finding was obtained in the control group whose percentages were 34.4% and 50.5% respectively. Anemia was present with the largest age group of 36 to 39 years with 36% of cases with mild anemia, and 29.9% with moderate anemia. Furthermore, preterm birth occurred in 19.1% of the pregnant women analyzed. The statistical analysis, performed with the chi-square test (p<0.05), showed a statistically significant association between anemia and preterm birth in pregnant women with extremely old ages
